With the washers leaving the paint on might be best, give it something to bite into.

I hear your pain, I'm still in the same boat with the rear slipping on my 29'er. I've mod'ed the tensioners a bit which seems to help, I got things tight on the drive side and not slipping. Bad news is that the non-drive side is not slipping back, not forward, so the tensioner is pointless on that side. And I had to keep my chain so tight, the only way to get my rear wheel off is to break the chain each time. This shouldn't be that hard.

I don't think I will ever be 100% confident of slippage.

If you're still running a qr rear wheel try a different qr. Some have a better interface with the frame(more teeth where the qr contacts the frame)

I appreciate the offer, but unless you have a supply of different types of chain tensioners to try, I'm not sure you can help me so easily. Could I get by with a couple serrated washers, sanding the paint out of the dropouts and maybe some brute force, maybe...the issue is, after the fall (where my wrist is still a little sore a month later), I need to be ABSOLUTELY positive that shit isn't going to happen again before I can ride this bike with confidence.

It sucks balls, but it 'tis what it 'tis.

I do have one or two different chain tensioners at home.

Got a couple suggestions, one would be to use a chain tensioner like the one used as a single speed conversion. (Assuming you have a derailluer mount)

The other suggestion is to run a front chain guide that has an adjustable position roller in it, you can run the chain through then slide the roller down to tension the chain. That would allow you to slide the axle all the way forward and if you had to change a tire you could just loosen the roller a little.
